Advance Care Planning Session

Cowichan Hospice is hosting an Advance Care Planning session on May 24, 2018 from 2:00 – 4:00 pm at the Hospice office. Attendance is free, and space is limited. Please call Cowichan Hospice to register, at 250-701-4242. 

Advance Care Planning is a process that supports you and your family to prepare to make decisions about your future health care. It involves understanding and sharing your values, beliefs and wishes regarding health and personal care. This information is used during conversations with health-care providers about the treatments and care you receive, to help you get the care that’s right for you. Make your health-care wishes known to your family and health-care providers before its too late. This will help ensure your wishes are known and respected.
